Not sure what rules you like to know, but you print out the visa, show on request at check in, show at Immigration at airport in Thailand. You will get a admitted until stamp with a period of 90 days and just before the last day you leave the country and come back for another 90 days, You can do this until the valid until date mentioned on the visa.
To clear this up. In the picture question nr 10 which you get when you apply for the Non Immigrant O visa. It clearly states: " for Coronavirus Disease(COVID-19)"
I understand it is very confusing and it seems that the Ministry of Foreign Affairs did not update the questionnaire attached to the visa applications.
I would suggest, copy and past the pop up, or picture or the way you are more knowledgeable to use computers, and put the pop up as answer of this question.

If you did not get ask to bring any documents, go with the passport and the attached document. Also bring the bill that you paid 2.000 Thb for the conversion.
As these document are asked at the Thai Embassy where you apply, the best option you have it to contact them and ask the same questions with the same explanation.
With this visa you get a maximum of 90 days, to be prepared to leave before 91 days after arrival.
The date 4 July 2565 is correct according the Thai calendar.
You state it is a different address then where you stay, Immigration is not making house calls to check. Ask again why there is a different address used, you have enough time left to apply for the Extension of Stay.
