The TM30 form is a form with number 30 from Immigration used to register you, if you did your registration online, you do not have to do it again in person using a form.
If applied for 1 year Extension of Stay the Immigration Office put a small paper in your passport with the text that you have to report your stay after being 90 days in Thailand, report date is on that paper.
With the new system it is possible to make your first report on-line, but some people still have some troubles with it. Try out as the window to report is 15 days until due date if using on line and if it does not work out for you you can visit the Immigration Office between 15 days before and 7 days after due date.
NOT make only a report if you go in person but also ask WHY the on line system does not work for you and have that fixed.

You can use the on-line report system after you made an account on this website.
You make the report between 15 days before and due date.
At the end of your current 1 year, during the last 45 days you can apply directly for a new 1 year Extension of Stay if you still meet all the requirements.
Ask the hospital to go in your name with your passport, 1,900 Thb and a hospital letter that you are unable to travel due to quarantine to Immigration and apply for a new Extension of Stay.
Or if the hospital does not give that service, ask a friend.
But the entry of 60 days with the visa can also be extended with 30 days. May be a hassle for you but there are people who like to stay maximum 90 days.
