your 90 days, which is a report that you're 90 days continiously inside Thailand, it stops at the moment you leave Thailand and it starts at the moment you enter Thailand. You need to calculate the days by yourself to make the first 90 day report.

No, you can only get once a 30 day Extension of Stay based on tourism when you enter Thailand without any visa.

You might apply for another 60 day extension of stay based on being unable to travel.

You apply for the Extension of Stay based on being married with a Thai National at the local Immigration Office that serves your address.

You need the marriage certificate next to other documents, but there is no requirement to bring a birth certificate.

Best is to contact your local Immigration Office and ask the hand out with requirements.
