You will want to talk to your Agent... And you will have to use an agent. For me it was 14 days + 14 days. Kinda of weird they told me come back after 14 days and they'll give me my passport with 90 day visa. After a that- come back and get visa for 12 months. But they called me, first time after 14 days then like 10 days after that. I hope they're more organized now - but this is Thailand.

That's the Indian or the Chinese vaccines? I personally don't care. I feel pretty safe here in Thailand. I wonder about other governments and will they accept the any vaccines?

I have never heard of a name verification before. There are many things I haven't heard of, so that may mean nothing.

I have traveled quite a bit. I never re-submit a TM30 for each new location. I think there maybe an issue if you TM30 in location A, but then try to do extension, or other immigration business at location B.

I vote no. The last time I looked (years ago) Airlines required Visa OR return ticket. Immigration's requirements vary depending on your visa. If you're coming on a long term visa that you satisfy Airline's requirement. If you have acquired a visa, then you have satisfied immigration requirement.
