I wonder if you made your own Facebook page, "your opinions and your opinions about other's opinions" would be very popular. I am not saying people are interested in your opinions, I am just saying they're so awesome probably they deserve their own Facebook page.
I am familiar with Tod Daniels and he has given me rock-solid advice in the past. But he is not the second-coming, He is human and sometimes doesn't get everything right every time. I always consider his advice, when available, but I don't take it as the gospel truth, after all this is Facebook.
I have had some difficulties opening a bank account at one branch and trying to conduct business at another branch. I think some people have not had these difficulties.
Hence, you will likely want to open account at a branch near you.
I used my driver's license as proof of residency. Different visa type, different requirement (residency vs location) but I suspect proof of location will be defined differently for each embassy.
You're pretty much guaranteed it will change in the future, just as it's changed in the past, so I am not sure what's your point?
"And you got warned that this would be the last time they allow you to enter without a visa that fits your purpose" I have been pulled aside twice, had the talk and never did they warn me 'this will be your last'. This is Thailand, they say things like "next time you come".
I used imagine editing software to make all my images into one image. I included a note explaining I was traveling around so my inbound flight is from some place other than my home country. It seemed to work. But if they've threatened you to "get it right" I would try to contact them directly by phone.

I imagine 'wet signature' mean notarized. My employer is cool but not cool enough to do additional paperwork, especially if that paperwork needs to be notarized.
Are you sending in hard copies or are they accepting all your documentation via web portal (are the accept eCopies?) ?
