Depends on the embassy, but as a rule you need to be in the country of the embassy you're using.

I'm sure people have been, I just haven't seen any. Normally though in that case the embassy just asks for additional documents , weird they would deny him unless he was unable to provide 6 months after they asked for it.

Old people can't cook? Beside, for the visa you don't actually need to do anything , it's not an ED visa. You don't habe to go to class. Just demonstrate an intention to study.

It's just like a Multiple entry tourist visa. It's valid for 6 months with 60 days per stay each entry. You can extended your 60 day entry ONCE in country.
