 You need to be on a Non-Immigrant visa to get a 5 year license. If you renew on a tourist visa, you'll get another 2 year license.

I've seen people do it. As part of the application you have to submit a ticket leaving the US going to Thailand. As long as you make it past that part (refundable ticket?) it doesn't matter where you actually enter from once the visa is issued.
