TLDR : Answer Summary
To extend a single entry Tourist visa at Chiang Mai immigration, you will need: a copy of your passport photo page and current visas, 1,900 THB fee, TM30 form, and forms TM.7 and STM.2. It is also recommended to bring a passport-sized photo.
you ask landlord or host to print a screenshot from the TM30 registration website

Ok, I know the place. It is a room at the far back left corner. It is helpful for me to know the order of what to do so that I do not waste time lining up only to be sent somewhere else and then have to start al over again! Thanks!

So they need a hard copy, so of course rather than having mine, I need to get them a hard copy BEFORE I get a queue number?
there's a little tent/building on the right side behind the main building. I believe that's the TM30 room. Or just ask one of the guards where TM30 is. You'll want to hit that before applying.
Chiang Mai 100% needs a copy and then you have to take it to the TM30 office at immigration and convert it to a hard copy on their form before you can submit your application.

Oh yes! Thank you. Do you know if I need both forms because usually they only have one?
