24 is a lot. Generally from what I've seen, unless it's recent and violent it won't affect you. I helped someone a few months ago get a Non-OA in the US, he had an felony armed robbery conviction 20+ years ago and they didn't care.

11 years without any issues I'd assume you're fine, only way to know for sure is apply and find out though.

28 is before 29. Steve did his after the due date and it wouldn't accept it. In person 90 day reports can be done 14 days before the due date, or up to 7 days after without a fine. Apparently the online system won't let you report after the due date though.

Visa exemption duration varies depending on nationality. 13 countries have 90 day visa exemptions. Other countries like China and India are not eligible for visa exemption and must buy a 15 day visa upon arrival, which is not extendable.
