Are you asking about the TM30? Any house/home/hotel/condo/apartment that houses a foreigner for more than 24 hours needs to submit a TM30 to immigration (either online or in person). Most immigration offices ask for proof of a TM30 when you apply for extensions. You'll get a little receipt, make a few copies of that and bring them with you.
So as Stuart stated the US embassy does not issue affidavit of income anymore, which is what you wanted. For anyone else specifically wanting a certificate of residency though, yes the US embassy issues them for $50, you need to make an appointment online.
If you're in Bangkok, immigration will only issue CoR's if you're on a non-immigrant visa, so getting one from the embassy is a work-around.

The only time you need proof of your TM30 receipt is when you go to immigration for an extension or a certificate of residency. Some people have landlord that just won't do TM30s for them, others use one from a hotel they stayed in 3 years ago. It's really not that big of a deal, and it's illegal for the landlord to charge you to do one. Technically if you have a friend stay over, he should submit one for you friend too. Tell him stop being a lazy **** and go file one for you.
