So because of covid, everyone entering Thailand right now requires Covid insurance for atleast the 1st 30 days. You can use travel insurance, the certificate must specify they you're covered for covid treatment in Thailand up to $50k usd.

Separately, the Non-OA visa requires health insurance. If your policy covers covid treatment (which it does, they all do) then you can use it, you just need them to send you a letter/certificate stating the above.

No, that is not correct. Visa exempt entries can always be extended 1 time for an additional 30 days in country.

1&2, just upload the first picture of your passport, the application is the same for everyone and those questions aren't relevant.

3: no, you need to upload the insurance certificate from the insurance company listing the amount covered and duration.

AKG in Chiang Mai is free, and takes 3-5 minutes for the results.
