That’s gonna depend entirely on the office they’re using. Some offices require a new TM30 when returning from abroad or even if you’ve stayed a night in a hotel in country and returned.

There are fields for departure info but not mandatory so ignore them.

You'll rarely be asked for an onward ticket at immigration, but it's possible. Usually, the issue lies with the airline, as some insist on an onward ticket within the allowed entry scheme days, while others don't seem to care.

TDAC has fields for departure date but they are not mandatory as it’s an electronic arrival card. Just leave them blank.

says most immigration offices don’t want to do the extension. Adding to that for your part 3 question, Phuket requires at least 15 days left on your current stamp to even consider doing it. Other offices may vary.

The rule is 2 months prior to application, leave at 800k or above for 3 months after application and can’t go below 400k for the rest of the year.

Two weeks is ample time for application. You could even do it on the 28th - but not recommended.

Go visit your immigration office and ask for their hand out of what documents are needed (can vary from office to office).

Most likely when you apply you’ll be given a “under consideration” stamp for 30 days -during which time they’ll probably arrange a house visit.

When you go back after the 30 days you’ll be given the remaining 11 months stamp.
