You should check with the Thai embassy in the country you apply or on the evisa portal, in Italy it is required.

It largely depends on where you are applying, usually on the embassy website you will have instructions, in Italy for example for a METV they require at least two confirmed entries (and exit) in Thailand.

your wife can piggy back on your extension of stay, no additional funds required. Once your extension of stay is done then you get hers done.

Visa and extension of stay are two different documents issued by two different departments (MOFA and Immigration) the embassy issues a visa for a determined period of time and within that period you have to leave the country, the fact that you may be able to extended it is not a factor in giving the visa in the first place.

If you come with 30 days visa exempt you need to show a departing ticket from Thailand within the 30 days or you might be denied boarding, that plus having to go to immigration makes it worth to get a 60 days visa. For accommodation, first is usually enough or just make one that you can cancel, plenty available.
