No. For an O-A Visa, you must pay for insurance from a Thai insurance company. The immigration website has a list of the approved insurance providers they will accept.

Your 90 day report date is independent of your visa. Your new 90 day reporting date is 11 September.

The Thai authorities will deduct a tax from the interest your deposit earns. Can't remember the %, but it isn't very much. Your principal will not be taxed. I'm American, if that helps. But, your nationality shouldn't matter, it's a tax on the interest.
