Yes, you usually need your original passport, filled TM47 form (90-day report form), and valid TM30 receipt for a 90-day report in person at CW.

Not sure what you mean by "based on Family", though anyway, no 30-day extension is available for Non-O entry.

You have to apply for 1-year extension based on the same reason as your initial visa by meeting all requirements.
