I got one today when leaving and didn't even realise I was able to, as I was leaving on an expired multi entry NON IMM O visa. The immigration lady pointed it out to me. Good friendly service.

Last report I heard was that they would only issue single re-entry permits. But that was from a while ago so I’m not 100% sure.

You can, as long as you are leaving Phuket by international flight(s). The re-entry counter is after security screening. Ask the officer around if unmanned.
