You must use your visa within 3 months of being issued . Then you will be stamped in for 60 days .

So you must arrive before it expires in Decmber xx as it says on the confirmation

There is no 45 day visa exempt anymore . That finished in March 2023 . And yes the day you enter is considered as day 1 regardless what time you entered
