lol you dont need a re-entry to leave the country at all so how about you give the correct info as well! As per its name a re-entry permit allows you to re-enter not leave.

I have DTV in old passport and have to show both new and old each entry.

Obviously different for you but try showing the photocopy of old along with DTV PDF and explain what happened
