if you live in Thailand you have to fill it out, even if you are exempt from paying taxes.

it depends on the case. In your case a marriage certificate would suffice, however if a Thai is visiting family (brother, sister, parent, etc) a birth certificate is usually required to verify the relationship.

I'm guessing you by visa status mean visa exempt stamp, and by round of 3 you mean the max 2 land boarder crossings per year?

Go to the police station with your passport and they give you a piece of paper that you take to immigration who then give you a new departure card

I just went there two weeks ago for an ED visa as well, feel free to ask if there's anything you're in doubt about :)

You can go to Thailand on tourist visa, but you have to leave Thailand to change to an ED visa. You can go to laos for example to get the ED visa
