I did, I came to Thailand first when I had chosen the clinic to use, they wrote a letter explaining what I was being treated for and how long etc, I'm get my menopause treated, do it's something that's ongoing for me for the rest of my life, the embassy also asked for the medical clinics business certificate as it's a small clinic, not a major hospital or anything, I was approved, that was just over a year ago through Canberra Australia

Dress respectfully when going into government buildings, definitely cover your shoulders for the photo
