I understand it is easy enough to obtain the form at Immigration checkpoint at the airport in Thailand.

I reported loss about a year ago at thong lo. No big deal. Not much time.

Using online for the first time, I was able to submit one easily and unable to submit the other with same arrival info though, of course, different personal info.
