I just received OA approval. The insurance requirement is an 'or' situation. For me I purchased one year medical insurance from a Thai approved company starting on the day I arrive in Thailand. The entire year coverage is purchased up front. The process was very easy, the Thai Embassy website provided a list of eligible companies and once purchased I received all required documentation within one day via email. I made copies of the documentation including the membership card and brought to thailand. This was important because immigration asked for proof of coverage at entry point entering Thailand. My copied documentation was accepted and I was stamped for the initial year. Hope this helps..

Submit a new application, make the correction, and submit same documentation. Seems that's the only option.

I believe the spouse is eligible for the SS amount that is greatest between the two spouses. So if your SS is 1000 and your deceased spouse is 1500 then you would get 1500. I might be wrong but you can find out with a google search. Of course SS only kicks in when you reach eligible age. Good luck.

A few years back I applied and got an OA visa via LA embassy. I got all documents notarized to include pokice background and medical.
