I have an health+life insurance gold (in and outpatient) with your Co. for 2 years now. I assume accidental treatment is included already, right? i was never told by the agent that I need additional extra accident insurance

To be honest I don‘t know, never read the letter itself. The bank knows what kind of letter immigration needs

It is a seperate print out from the bank where you get the letter from at the same time. Of course the bank book mist be updated to the day of application, so just make a withdrawel or pay in.

If you need it for EoS then ask your immigration office. Every province has its own rules, some say same day of application others grant 6 days

What issues will I face at immigration when leaving Thailand if my second 90-day report is overdue?

If your 90 d report was due on 22nd Jan and you tried to do it online on that day, then it was rejected because too late for online reporting there is a period time given: between 15 to 7 days before due day. You can‘t do it online on the last day.
