I know that TM30 is required but I am not sure if you have to submit a copy of it when doing a 90 day report. Having a copy with you would be the way to go.
Also copy of entry stamp if not shown on the copy of the visa stamp. Front page of passport actually means data/ photo page which isn’t the front page on some passports ( eg UK). I don’t know if a copy of TM30 is required but you need to make sure that it has been submitted.
Note that when you cancel the stay extension you might have to leave Thailand the same day. If you need some time to get your affairs in order you can fill out a TM7, pay 1900 Baht and you will get a stamp stating that your application for extension of stay is not approved and giving you 7 days to leave the country. The last time I transitioned off a work permit my employer HR went to the Labour Department, cancelled and returned my WP obtaining a receipt then went to immigration and cancelled my stay extension then applied for another stay extension which got me the 7 days to leave. Make sure you get a copy of the WP cancellation document if you think you might want to work here again sometime in the future.

UC until 25 July ( 21 days). I assume that Bangkok immigration has the capability to do a home visit if they think one is warranted based on the interview even though they don’t do it routinely.
Good luck with it. Preparation and organization of the documents is key to success I think. In my case the IO didn’t say anything to me she only spoke to my wife in Thai so I just listened in and it worked out fine.
Yesterday I gave them 2 copies of the 6 months transaction list and they asked to see the original to verify them so I am sure that it’s needed. I believe that it’s required so they can verify the 2 months seasoning of 400k. Your bank book might show rolled up ( summarized) transactions in case you don’t update it regularly so it doesn’t necessarily confirm that you meet the requirements whereas the transaction list shows every transaction. I did the letter, transaction print and passbook update in about 20 minutes at my bank for a total fee of 200 Baht so you might as well do all three at the same time. Don’t forget to pay in any amount of money to your account on the day you update your passbook so it has an entry ( and balance) on the day that you apply.
