It’s the equivalent to the old arrival card and the details should be where you arrive from which in your case is the layover in France so enter the details of the flight you enter Thailand on. Deep down it doesn’t matter but best to get things correct

I said this from the start because immigration told me there was no limit but they are tracking border run abusers and people in general that abuse the visa exempt system trying to stay there long term

Ted P E Tilden Jr. DTV is a 5 year visa and can stay for 1 year each time. Multi entry tourist visa you can get 9 months from but do have to do a border run after 90 days

I paid 4k for extension and TM30 with my agent in Jomtien. I did contact Grace but was a lot more expensive. I dropped my passport off one day with her and she returned it to me the next day. No queues, no questions, just convenience!

My mate submitted his on 13th December and got his DTV through on 23rd December. No additional paperwork was required so that obviously helped

This is what I never understand. When they introduced this visa it was stipulated that you can stay 180 days and then extend by a further 180 days but yet people have issues for sticking to the rules. I know many people that have had various problems! I know this is Thailand but why do they make everything so hard work?
