yes it is and many hotels are selling 15 nights 16 days packages but some are selling 14 days packages which are more correct. The exact line from thai immigration at this moment is that arrivals will have to quarantine not less than 14 days

At the moment you're vaccine doesn't matter! But I expect this will change soon.

At present time it's 15 nights quarantine. But if you have to go you have to go. I've just completed everything apart from negative tests. It's been hard but?

Thailand embassy does not require insurance for your visa. This application sounds complicated. You fill in visa online with all required information. Pay online post your passport and payment receipt (one with barcode) also a special delivery envelope addressed to your self to receive your passport back. That's it. Don't know why yours is taking long time. Good luck for you!
