How can I secure a visa extension appointment in Thailand if the booking system is full?

The appointments are only opened for 30 days in advance... since they are all booked, tomorrow you will see appointments open for 20 Dec only, until they're all booked, then you have to wait until the next day to see any available for 21 Dec.

DTV is great, but remember, it is essentially an extended tourist visa so things like opening bank accounts, getting driver license, purchasing vehicle, etc., may be a bit of a challenge.

 very true… here is a snapshot of my recent monthly charges from MEA… I travel a lot, so the months that are 200+ baht I was not there, the months that are 1600+ THB, I was there for about two weeks out of the month, and the ones that are 3000-4000+ THB, I was there the entire month.

 mine is usually between 3000-4000 THB/month…. maybe because I run a/c most of the time, cook daily, have computer running, etc
