There are similar posts on other forums with people having the same problem in Chrome. The answer was not to use google translate (for whatever reason). This worked for many of those having this issue. The other "easier?" way to do this is via the app instead of the website... this is much much easier and far less prone to error.

If the stated goal is to have 50+ long stay visitors in Thailand covered by health insurance, then it makes no sense at all to limit this specifically and only to O A visas obtained outside the kingdom and extensions of those O As, then ignore all others over 50 on other types of visas/ NON O, and in doing so, create a clear loophole for those over 50 to skirt the stated goal. BUT as far as this police order is concerned, the above is complete speculation.

Thailand Elite is wholey owned and operated by TAT - Tourism Authority of Thailand. It aint going anywhere... on my second PE visa with the new one being a 20 yr.

Thailand Elite will tell you that you will be on overstay from the date your membership expires regardless of the permitted to stay stamp in your passport.

... the way I understand it is that the bank letter can be from the day before (if that's when you're getting it).. the bank book update is the day of the extension... which can be done in the basement of chanwattana via the ATM assuming your account is one of the supported banks in the complex.

Robert is correct, in addition (at CW at least) you will need to make sure your current TM 30 matches the address of your last 90 day report

there will be no fine since the requirements are for notification within 24 hrs of entering the country. Since you will be doing a border bounce (which means you will have a new TM6) and presumably reporting within 24hrs of entering the kingdom, no fine.

Apologies, I though you were from the UK. I do not know if you are able to get a residence cert from US embassy (many countries dont .. including mine) You're going to have to speak with your local bank first to see what hey require (many branches reqs to open an account vary wildly from one branch to another)
