That’s the validity, not the duration. You have to enter within 90 days. Then you get a 60 day entry

I got my DTV in Jakarta:

- current address > my address in Jakarta.

- Permanent address > my address in europe

- Date of previous visit to Thailand > my last visit to Thailand (didnt list every visit)

- Countries for which travel document is valid - in my case Norway, as I have norwegian passport

- Name and Address of Local Guarantor in Thailand > BLANK, not required for DTV

The form is generic, so fields not related to your specific visa can be left blank,
