I just paid the normal amount for the treatment. I don’t think there’s a minimum required.

I personally would make sure the letter says you have it over a period of months. 

I booked two treatments over two months both work on my back molars. It was a dentist I knew in Chaing Mai.

I’ve previously posted I just got him send me a letter on official note paper confirming the treatment over two months that may extend longer.
