There's no embassy to visit inside of Thailand, that wouldn't make sense.

You can get one extension each entry, in theory. But immigration doesn't want anything to do with the DTV so they purposely make it so difficult you won't want to do it. Just leave and return every 180 days.

Each bank branch manager sets the requirements for their own bank branch. So you'll need to keep trying other branches until you find one that will do it. A yellow book should not be a requirement as it's not possible for everyone to get one.

You said you got a TM30 but that doesn't do anything for a bank account. Have you gotten a residence certificate from immigration that specifically says it's for opening a bank account?

You only need health insurance if you entered Thailand with the 1-year non-OA visa that also required health insurance.

For a non-O extension, no health insurance is required. A medical certificate wouldn't be required for an extension with either of those.

You should go to your immigration office ahead of time and ask them for their handout of requirements, which will list everything that your specific office needs. Each office has their own requirements.

Also you should check your calendar, because January 3rd is a Saturday. You do not have to apply 45 days before your stay expires. You can apply when you have 30 days or less left at most immigration offices, and when you have 45 days or less left at some offices.

The agent might not be able to use Bangkok Bank for extensions anymore, but that has nothing to do with you.

You apply for the non-O visa from the Thai embassy before you travel to Thailand using your personal bank account in any country.

Then once you're in Thailand you use the non-O visa to obtain a bank account that you'll need for the extensions.

Banks will not care anything about a birth certificate. That has nothing to do with a bank account.

Not everyone needs to go. 90 day reporting only requires the passport and the completed form. The person for the report is not required. Anyone can do it for you.

you must be on the status you are extending during the time the transfers were made in order to use that method to extend. Which means the transfers must have all happened while you were on a retirement visa/extension already in order to use it to apply for an extension. Therefore it's not possible for the first extension. Even if you have 10 years of transfers, it will not be accepted. Only embassy certified income or 800k in the bank.

yes. But best to check with your immigration office as some might have further requirements to use that method like proof it's a pension.
