apparently they just go to the Embassy on the day of the appointment. Turn in the documents and the Embassy tell you when to go back to collect your passport.

You've got a field required in 7 because at the beginning of the application you have either entered Thailand or the UK in the field asking where you have residency. You need to leave it blank
