The city where the passport is issued should suffice . I enter London ( that is the site of one of several UK authority offices) , both on-line and manually on immigration forms and have not experienced any problems. However, e-visa applications made outside of Thailand sounds like a different scenario ? Also what country is the OP from ?

- 1st May is the start date being quoted, hence why myself & others are asking so many questions . You have placed the post with some information, so I am guessing you must believe this will be happening redardless of no official announcement? All matters that may affect expats will of course naturally attract debate and all types of inquiries.

And what happens to those people who don't complete the form or do so incorrectly, so it's rejected?

If you want to report in person , you can go 2 weeks before the 14th April and up to 7 days after that date. So you should have no problems whatsoever submitting your 90 days report.

Take Todd's advice and go to Jomtien IO and at least a week before you submit your application. They should supply you with handouts , both in English & Thai of what the requirements are for their particular office. If not, will have the list of requirements advertised somewhere on the walls of their office, so you can take photos. Every IO in Thailand has it's own variations of the rules for extension applications. As it's your first extension, be best if you and your wife go to Jomtien IO together, as you may both want you to also ask questions at their information desk.
