You either get it in immigration office or you can download it from the Thai immigration website. It just states your arrival date and address.

apparently I have to! My visa stamp is till February 27th but as I went 15 days earlier for this report my next 90 days report is on Feb 9th. If you wanna avoid that, and only report once in 180 days period, come right at the 90 day mark or a few days later. I know that for next time!

The visa is valid for 5 years. There is no re-application. Extension is 1900 baht.

Just walk-in. I went 11:30am they close at lunch for an hour. And was still enough time to get everything done.
