That depends on what the issue is. If you made any mistakes on the basic information from your passport like name, passport number, place of issue etc it is an immediate application cancellation.

If you just failed to submit adequate supporting information, they will most likely contact you for additional information.

Multiple posts about this for Chiang Mai. It's a process that the CNX airport has implemented for all DTV arrivals.

A tourism extension would be added to the end of your existing permission to stay.

No there is no template. You need to write that you will be financially responsible for them and that you are their sponsor for their DTV based on having a DTV yourself and that they are your dependents (children, wife).

Not even remotely reasonable, since you still need to provide all the paperwork. A non-OA visa can only be obtained from the Thai embassy in your home country, and you would need to provide all the required paperwork, including medical certificate, criminal background check, proof of insurance, proof of funds. What would an agent do that can assist in that except take what you provide and upload it into the e-visa system?

Unless you are mistaken and not applying for a non-OA visa at all.

Neither of these is required for a non-O visa. Only a non-OA visa requires these, which can only be obtained from the Thai embassy in your home country.

With a multiple entry visa, you get a new 60-day stamp EVERY TIME you enter Thailand. Getting a re-entry permit would actually give you less time in Thailand. Also you cannot purchase a re-entry permit at land borders.
