I sent them a copy of the appointment slip the hospital sent me. It’s got all the important information on there. Facility, Doctor name, specialty, my info, etc

no payment upfront. It’s a consultation for possible treatment of a medical condition. I’ve used the Thai medical system before. Excellent facilities and treatment

Yes I was approved using Thai Soft power. I used a medical appointment. I made the appointment online and used the appointment slip as documentation. No problems. I applied in U.S. and used the e Visa through the Chicago office

yes. They have many locations throughout Thailand. I used the one in Thong Lo and Chiang Rai. Great care too!

I had been to Bangkok hospital on my previous trip to get some lab tests. I just requested another appointment through the website and they sent me the appointment slip. That’s what I provided to the embassy
