Robert Clifton Dailey the US Embassy isn't vetting it. They have no need too. Immigration are vetting it by requesting the document's to back up your affidavit. Nothing wrong in that, they are entitled to ask for proof. They always have been.

No. They don't ask to see it. They can see on the system you have already filed one. In fact, all you need at Jomtien is your passport and previous 90 day report slip.

I think you'll find Vietnam want the same plus proof of funds in a Bank account.

KL may not even give the visa on the presumption that your going to extend another 30 days. Penang won't. I'm not certain about KL but the onward travel may need to be within the 60 day visa validity.
