Generally you will have to merge several documents together into one for upload. You can include a cover letter in the merged document if you wish.

Sounds like they just transferred your current entry stamp, not anything to do with the DTV. Not particularly useful.

Take photos of every page of your passport just before you leave and also a selfie in front of your hotel, hotel keycard/address etc. in case they ask for it. There is a small chance your application will be rejected if they dig deeper and you will have to apply again. You don't really need to go back after if you don't want to, if they ask you in for an interview, you will fail it anyway because you will have an exit stamp in your passport.

The loan is denominated in USD, the collateral moving in price doesn't affect the loan amount. Only thing to be aware of is the interest rate and provide enough collateral to not get liquidated on a down move.
