×Register all that information + your vaccination certificate with Thailand pass.
Also, if your holiday is less than 30 days you can arrive by visa exemption if you're a Netherlands passport holder so you don't have to worry about applying for anything in that regard.
If you apply for the visa beforehand make sure you apply with plenty of time to spare to avoid the stress I went through lol. I applied 2 weeks before my flight and almost didnt get it in time.
It's the discretion of the border agent, but mine looks like shit and I've had no problems all over the world. As long as it's not on the ID page I think you'll be ok.
