The clinic would be who you need to speak to. Ask them if their treatment has already been used to apply for a DTV. What documents they’re able to supply to support your application.
If they haven’t supported a DTV application yet, ask them to research it and get back to you

Maybe whoever’s in charge of immigration at the airport has decided on this particular stance with DTV holders? Who knows. As you know there’s so much leeway given for interpretation of rules, in all sectors, not just immigration, that it’s common to see these variations in service levels and requirements all over the country.
Anonymous participant fair enough but it’s one of the most expensive places in Thailand. In the medium to long term, I’d advise you to start looking elsewhere.
