USA generally within 10 business days. LA has been very fast recently approving some in 2-4 days,

You have to do the 90 day report if you styed 90 consecutive days. The first time must be in person at immigration. You can leave and enter as you please within the 180 days limit.

As a DTV holder you should have no problem reentering. Your past record of border runs is the perfert reason to get a DTV.
