Check-out date is only for your guests. The registration of the house does not require a check-out date.

Immigration Phangan mostly asks for TM30 when extending the exemption.

You need to do the TM30 registration for yourself anyway (check google). Sometimes immigration wants to see it before extending your stay for another 30 days (฿ 1.900).

 You can extend the 45 days of visa exemption for another 30 days and ฿ 1'900.– at immigration in either Patong or Phuket town.
