fortunately my passport had a issuing authority field and Bangkok was written in there. So even tho the form may ask place of issue, I found putting the name found in the issuing authority field to work for me, and it was also the embassy suggestion for the place of issue.

Anonymous participant there is no 90 day TR visa, it's a 60 day TR single entry visa that can be extended once an additional 30 days.

Possibly the passport place of issue?

What exactly does it say in your passport, and what did you enter in the form. Don't forget that the photo upload of your passport data page can override the data you manually entered, you must double triple check it and edit if necessary before submitting.

depending on the local office policy, usually if you have a primary residence with a TM30 filed and travel around Thailand, then return to your home many offices don't care, check with that office about their policy, if you truly are moving you need your new house owner to file an updated TM30.

the terminals are your last resort if you didn't file before arrival. You'd be sent to the terminal before you get in front of an immigration officer, fill it in, get your TDAC approval sent to your email, or possibly also scan a QR code, show the officer your approval QR code.
