Have you considered not flying via Bangkok and flying into Chiang Mai instead? You're coming from the US, which means you'll have a layover in East Asia and you can fly directly into Chiang Mai from mainland China, South Korea, or Taiwan with a full-service airline. You can also pay excess on discount airlines like HK Express if you fly through Hong Kong.
That's a single-entry tourist visa. A multi-entry tourist visa is valid for six months from when it's issued. Op can enter and exit as often as they please in those six months, getting 60 days per entry, extendedable by 30 at immigration.

an educated guess - your Phuket TM30 isnt in the computer system yet. Depending on the office, they can take months and month to appear. Wont be a problem in person.
