Yes, things changed last July when Visa Exempt went to 60 days and the availability of the DTV hasn't helped

You maxed out a Visa Exempt stamp and got 90 days which many Immigration Offices so not see as typical tourist behaviour

As Brandon says below - Get the visa from your embassy before going to Thailand. That will save you 2 trips to immigration and you'll be able to apply directly for the 1-year extension from that.

What does it mean if my VOA extension stamp says I need a re-entry permit before leaving Thailand?

As I said this stamp is a generic Extension of Stay stamp with the standard wording. All Extensions of Stay require a re-entry permit to maintain the status.
