Is the T8 Health Declaration Form Still Required for Entry into Thailand?

I'm used to the arrivals and departure card that the flight crew give out but the document that Qatar link you to has about 40 health pages one of which is labeled T8 and asks if you have been vomiting etc. I'm assuming its only the T8 but all new to me!!
