TM8 IS the application form for re entry permit for those on retirement extensions etc. Not needed for tourism. The T8 seems defunct now Thailand Pass is in operation. At Thailand pass check in at Bkk Airport there is a list of things which there ask if you are suffering with. Similar to those on the old T8.
but I can confirm. I just landed in Bangkok a couple of hours ago and the T8 form wasn't given by the airline attendants neither asked for upon arrival.
that's a different for altogether t8 is health form the one you thinking of is the blue landing card

I'm used to the arrivals and departure card that the flight crew give out but the document that Qatar link you to has about 40 health pages one of which is labeled T8 and asks if you have been vomiting etc. I'm assuming its only the T8 but all new to me!!
