The Thailand pass now includes your T8. apparently so its all in the QR code, never worked that out as they ask you for your seat number.
Its not a new form I always had to fill one in and report to the health desk when i did jobs in west africa

I got it in Singapore yesterday before departing to Bangkok…like it’s been done in the past or on the plane before arriving immigration definitely want it and you get the “departure” part to keep
