Look up the procedure on the embassy website. In ChiangMai they are very strict about making appointments, and you have to apply online for the appointment.

When you show a bank statement, what form is that in? Are they looking for a printed statement on bank letterhead? Can you log into your account on the phone and show them the current balance? How about an ATM receipt showing the balance?

Chris Bugov Bangkok Bank is the only one that will allow you to open an account with a tourist visa, and even then there may be questions. I got the best service at their office in Central festival mall.
