sure no problem. If you feel compelled you can upload in the section where it says supporting documents.

You don't have to anymore. It was required earlier when they started letting people apply for visa's again but no need to upload anymore. Fill it out just in case any officials asks for it. But when I arrived 12 days ago no one asked to see it.

to be honest it's not worth it if you are only going for 30days or less because you will spend 15days in quarantine

originally only for low risk covid countries but the Cabinet approved the motion to allow all countries to apply for it now. It will take a few weeks to roll out I'm sure.
